The Create Meta lead form page is a tabbed wizard covering every field the Meta leadgen_forms v25 API exposes.
Wizard tabs
- Basics - ad account, Facebook Page, form name, privacy policy URL.
- Intro screen - cover photo (library / upload / AI generate), headline, paragraph, paragraph vs bulleted-list style.
- Questions - 22 prefilled types (email / work email / phone / company / work address / state / country / date-time / appointment request / etc.) plus CUSTOM (with helper text) and MULTIPLE_CHOICE (with custom options).
- Thank-you - post-submit screen with title, body, and a VIEW_WEBSITE / CALL_BUSINESS / DOWNLOAD button.
- Advanced - locale override, higher-intent review step, organic-post lead toggle, UTM tracking parameters, short legal disclaimer, full custom legal body.
Cover photo picker
- Content library - pick any ready image MediaAsset already uploaded for this brand.
- Upload - PNG, JPEG, or WebP up to 8MB. Stored as a new MediaAsset so you can reuse it.
- Generate with AI - type a prompt (or leave blank to use the brand-aware default template). Credit-gated; renders via kie.ai Nano Banana 2 and saves the result as a MediaAsset.
AI end-to-end draft
Click Generate full form with AI to draft every tab at once. Optionally add a website URL (fetched server-side with an SSRF guard), paste extra context (brand KB, product docs), and toggle "Include this brand's guidelines". Our AI returns a structured payload: form name, intro headline + paragraph, context card style, question page headline, locale, is_optimized_for_quality, allow_organic_lead, a cover photo concept prompt, a complete thank-you page, UTM tracking parameters, short legal disclaimer, full legal body, and 4-6 questions mixing prefilled + MULTIPLE_CHOICE. Check Also auto-generate the cover photo from the AI concept to have the wizard render the cover image in the same pass.
Permission errors?
Meta needs the pages_manage_metadata scope on the Page to publish forms. If you hit a permissions error, reconnect your Meta ad account.
